Insurance Participation

Cardiothoracic & Vascular Surgeons of Michigan participates with many different insurance companies, including most HMOs. If you are referred by an HMO or PPO, you will need to bring the appropriate authorization form with you.

If we do not participate with your insurance, you will be given a receipt that you may submit for reimbursement. We will make every effort to help you submit your claim, however, payment is expected at the time of your examination. If you have any questions concerning your coverage, contact us and we will be happy to assist you prior to your examination.
